Please register on-line, if possible.
This will help us capture all of your information correctly, prevent data entry errors and save volunteer time. Note that you can submit an abstract on-line. We hope to post these short abstracts on-line. If it works out well, we may even begin having abstracts in the program.

If you want to add or correct information, you can re-enter your registration information. Be sure to re-enter any previous information that you want us to have and add your additional information such as an abstract. We'll capture your latest entry and delete the original information. You do not have to pay twice, of course. If re-entering data and you are already paid, simply click "pay later".

Return paper forms by fax, mail or e-mail - see form for address. Print forms on your PC, complete, then fax or mail to address on form. To submit via e-mail, complete the Word version, attach it to your e-mail, and send it to the e-mail address on the form.

Note that the "Pay Later" option allows you to pay by check or PO. This option also allows your to re-enter your data, add a presentation or abstract without repaying. Be sure to re-enter all the data you want us to capture as we will delete the first entry except for the payment information.

Payment information is on the registration forms. Please DO NOT send cash via mail. If you send credit card information via fax or e-mail, please be advised those methods are not secure. You may wish to contact W. George Fong to arrange more secure credit card payment.
